Required Transcripts for Application

COTC must have all of your final, official transcripts from high school, College Credit Plus, college, and other institutions on file by the application deadline. If we already have the most current transcript(s) on file, you do not need to resubmit. 

  • Official transcripts must be in a school sealed envelope or sent electronically directly to COTC Records.​ 
  • International high school and college transcripts must be evaluated Links to an external site. by a current member of NACES and an official report must be sent directly to COTC Records.
  • Students who attended multiple institutions must request a transcript from each institution attended.​
  • Students who withdrew from a college before completing any courses must verify if they have a transcript on file.  If so, they must request a transcript.​
  • Students with continuing education credit or employment training provided through a college must request a transcript.
  • Students who attended a permanently closed institution must provide a transcript.  Please visit the Ohio Department of Education Links to an external site. website for information on how to request from a closed institution.​

Active military and veterans:

  • Transcripts needed may include CLEP, DSST, JST and Community College of the Air Force​.

  • Please reach out to us and we will have you work with Nursing Program Director and the Military and Veterans Resource Coordinator for required Military training and deployment paperwork.
